After searching a lot for a scheme I've decided to try to build at my own and there's my results. I've tested both cables (PS/2 and USB) on CRT and LCD screens and works very fine.

Comment: In my case pin 25 and 13 are changed. 13 is VGA RED GND and 25 is VGA RED

 

According to 3 reports in our database (2 positive and 1 negative) the Belkin OmniView MATRIX2 Series KVM Switch (F1DM208T, F1DM216T) pinout should be correct.
